Down Syndrome to You: Personalized Care Plan

 It is estimated that more than 95% of individuals with Down syndrome don't have access to a Down syndrome specialty clinic.

Researchers at the Massachusetts General Hospital created Down Syndrome to You (DSC2U) and are looking for families to participate in a trial of this new health care service to address this need.

DSC2U is a personalized care plan - caregivers complete an online form to identify current health care concerns. The response will generate two documents - a plan for the family and a plan for the primary care doctor.
